This teaching course covers both basic and advanced magnetic resonance imaging in vascular cognitive impairment (VCI). The spotlight will be on cerebral small vessel disease (SVD), the most common cause of VCI. The course will provide an overview of concepts and methods and starting points for exploration of emerging techniques. We will also point out major pitfalls in SVD neuroimaging.
We will first explore the classical lesion spectrum of SVD, both for ischemic and hemorrhagic lesions (presented by Prof. de Leeuw). Recently it has been discovered that these lesions are highly dynamic. They appear and grow but can also shrink and even disappear. The second part is an in-depth look into diffusion imaging, a major technique to study microstructural alterations of white matter in SVD (presented by Dr. Duering). We will cover basic diffusion tensor imaging as well as advanced diffusion modelling and structural connectomics. The third part is on emerging techniques offering pathomechanistic insights (presented by Dr. Doubal), i.e. measurement of blood-brain-barrier permeability by dynamic contrast enhanced imaging and assessment of cerebrovascular reactivity.

The VasCog 2018 neuropsychological testing session will focus on the interpretation of neuropsychological test data within the context of hypothetical constructs as related to brain and cognition, and; how to use neurocognitive constructs to help define specific phenotypic profiles associated with dysexecutive disorders and amnesia. The clinical application of these hypothetical constructs will be used to illustrate phenotypic profiles that typify Alzheimer’s disease, vascular dementia associated subcortical white alterations, mild cognitive impairment subtypes. Additional topics to be discussed are the integration of classic neuropsychological assessment paradigms with new digital technology, and how new technology may be used to better understand or disambiguate brain – behavior relationships.

Recent advances in in vivo optical microscopic imaging and single-cell biology has dramatically enhanced our understanding of mechanisms underlying the regulation of the blood-brain-barrier and neurovascular coupling. This teaching session intends to give an overview of basic research in neurovascular biology and introduce an array of basic science techniques including multiphoton microscopy, vascular mural cell transcriptomic studies to young researchers, with illustrations by experimental protocols and highlights from the speaker’s lab.

By bringing together international cohort studies of cognitive decline and dementia following stroke or transient ischaemic attacks (TIA) and other longitudinal studies of individuals at high risk of vascular cognitive disorders (VCD), the Stroke and Cognition consortium (STROKOG) aims to facilitate a better Understanding of the determinants and manifestations of vascular contributions to cognitive disorders and help improve the diagnosis and treatment of VCD. STROKOG has developed into a consortium of 30 studies from 18 countries in five continents, with a combined sample size of >16,000. The harmonisation project has been completed and the results will be presented at this special session by the STROKOG coordinator, Jessica Lo. The data show that the prevalence of post=stroke cognitive impairment is uniformly high across all cognitive domains. History of past stroke, and to a lesser degree, hypertension, AF, and smoking are associated w While some ethno-racial differences were seen, additional studies in non-White groups are required to further explore these differences in cognitive impairment.
