Registration Procedure

Please complete the registration form and return it with appropriate fee to the Congress Secretariat either by fax or mail.

Registration Fees

Registered delegates will be entitled to:

	Attend all scientific sessions on 18-19 April 2009
	Attend the Opening Ceremony on 18 April 2009
	Coffee breaks and lunch on 18-19 April 2009
	Delegate's kit and all official publications
	Access to exhibition area

Payment Method

Payment can be made by one of the following methods:-

1) Credit Card
Please provide your credit card information on the registration form and send it to the Congress Secretariat.

2) Cheque/Bankdraft
For overseas delegates, please send bankdraft in HKD or USD made payable to "The Chinese University of Hong Kong". Personal cheques are acceptable for Hong Kong residents only.

3) Telegraphic Transfer
Please contact the Congress Secretariat at cardiology@cuhk.edu.hk for details.

Cancellation

Notification of cancellation must be made in writing and sent to the Congress Secretariat. For cancellation before 31 January 2009, 50% of the registration fee will be refunded. No refund will be made for cancellation after 31 January 2009.
